A Guide for Building Effective Linkages between CARE Act-Funded Providers and Key Points of Entry to Health Care.

These linkage tools help agencies assess, develop, implement, and monitor linkages with other organizations that are either part of the HIV care continuum and/or key points of entry into the health care system for people living with HIV.

Below are a series of facts sheets and worksheets to help organizations develop effective linkages and referral systems. The fact sheets provide general knowledge about key topics. The worksheets each have two components. The first, in PDF format, provides guidance on how to complete key phases in the linkage process and the second, in Word format, provides electronic documents that you can use within your organizations to complete the task outlined in the corresponding PDF document.
